📋 Description Provides care to patients and customizes treatment plans. Coordinates behavioral health care with the patient’s PCP and other service providers. Screens for behavioral health and substance use with PHQ9, GAD7, CSSRS. Monitors symptoms and side effects to reduce symptoms. Supports psychotropic medication management by PCPs. Tracks follow-up and outcomes in a registry to re-engage patients. 🎯 Requirements Master’s Degree in Social Work, Counseling, or related field from an accredited program Licensed to practice in California as an independent clinician (LCSW, LPCC, or equivalent) Minimum 1-5 years experience as a licensed professional A secure, high-speed internet connection capable of supporting video conferencing and cloud-based systems Proficiency in English and Spanish medical interpretation and documentation Knowledge of/experience with behavioral health screening tools (PHQ9, GAD7, CSSRS) 🎁 Benefits 100% remote with employer supplied computer equipment $50/month non-taxable remote stipend Opportunities for Supervision Work/life balance with 10 paid holidays, sick time, and flexible PTO Medical, Dental, and Vision plans start on day one 401K plan
