📋 Description Lead efforts to improve HEDIS med adherence & other quality measures Analyze adherence data to identify gaps & implement interventions Collaborate with providers, care managers & stakeholders Conduct MTM sessions & educate patients on regimens Engage patients with motivational interviewing for adherence 🎯 Requirements PharmD from an accredited school Active pharmacist license in TX, PA, AL, TN ≥2 years in HEDIS-related adherence or population health Strong knowledge of HEDIS measures for adherence Proficiency with EHR systems & data analysis tools Excellent communication, problem-solving & organization Ability to work independently and in a team Experience with motivational interviewing is a plus 🎁 Benefits Work from Home: fully remote company Medical, Dental, and Vision plans 401(k) with 3% employer match Life & Disability insurance; voluntary life options Employee Assistance Program (EAP) Paid time off & parental leave
