📋 Description Manage DHS and CMS duties for MSHO/MSC+ members in the community. Perform face-to-face visits and annual assessments with members. Develop individualized care plans and goals with the care team. Coordinate with community resources and services; document accordingly. Travel 50-70% locally; remote work 30-50%. Hours: 8am-5pm Mon-Thu, 8am-3pm Fri; no evenings or weekends. 🎯 Requirements Current Minnesota Registered Nurse license. MN Licensed Social Worker (new grads encouraged). 1+ year experience with geriatric or disabled population in case management. Valid driver's license. Ability to read/write/speak English.
