📋 Description Manage DHS and CMS activities for MSHO/MSC+ members. Work from home with regular local travel. Travel ~50-70% in North Branch area; ~30-50% remote. Coordinate face-to-face visits and Elderly Waiver. Conduct annual PCA assessments for community and assisted living. Develop care plans and support transitions of care. 🎯 Requirements Minnesota RN license. MN Licensed Social Worker (new grads welcome). 1+ year geriatrics/certified disabled case management experience. Valid driver's license. Independent worker with private home office space.
