📋 Description Make home visits to clients in designated territories. Assess social and emotional factors related to health. Provide social evaluations and plan interventions. Identify community resources for client/family needs. Advocate for client and family through the community system. Document observations and interventions using EMR on a tablet. 🎯 Requirements Licensed by TN Board of Social Work Examiners at LMSW or LISW. MSW with at least 2 years of community-based experience. CSWE accreditation demonstrated by transcript or degree. Home care experience a plus. Strong English communication; read, write, and speak clearly.
