📋 Description Perform outpatient therapy evaluations to plan care for seniors in designated senior living communities or group homes. Deliver care at top of licensure using evidence-based approaches to maximize function. Coordinate with home health partners to transition care from home health to outpatient services. Educate clients, families, and caregivers on care plans to maximize independence. Perform evaluations, treatments, and discharge planning using outpatient EMR. 🎯 Requirements Current Maryland OT license. Prior home care experience preferred, but not required. Minimum one year of OT work experience. Graduation from an OT program accredited by AMA and AOTA. Ability to work independently and manage time effectively.
