Community Health Worker - Skid Row

📋 Description Manage a caseload of patients; complete assessments and care plans with patients and teams. Build rapport with patients to encourage positive behavior changes. Coach patients to minimize risks from identified health conditions and behaviors. Accompany members to medical and social services appointments. Connect members to programs addressing barriers to care and boosting compliance. Link members to local, county and state services; follow up as advocate. 🎯 Requirements High School Diploma or equivalent. Bilingual in English and Spanish preferred. 2+ years experience with CalAIM populations. Ability to work in outdoor, dynamic field settings. Independent and team-oriented work style.
