Behavioral Health Case Manager

📋 Description Serve as main contact for new participants and families during onboarding and journey. Advocate for participants joining the PACE program; ensure seamless onboarding. Facilitate care coordination from pre-enrollment to enrollment with IDT meetings. Review participant tracking tools to identify immediate needs; coordinate with IDT. Oversee monthly new enrollee orientation day, WelbeLink training, and center tour. Conduct checkpoints at 30, 60, and 90 days post enrollment and ongoing follow-ups. Partner with Behavioral Health and Social Work teams for high-risk participant check-ins. Identify high-risk participants for disenrollment and address concerns. 🎯 Requirements Bachelor’s degree in human services, communications, hospitality, or related field; experience may substitute. 4+ years in customer service or customer-facing roles. Highly motivated, self-directed; adapts quickly and makes decisions in emergencies. Proficiency in data entry and data analysis. Proficiency in Microsoft Office programs. Ability to manage sensitive conversations with compassion and discretion. 🎁 Benefits Medical, dental, and vision insurance coverage.
